Leonza Ucles
The Patient Referral team had visited Leonza during a home visit on Central Michigan University's brigade in Hoya Grande. Leonza is 101 years old and has been sleeping on a matress that was incredibely hard. Leonza was diagnosed with dementia, arthritis, and has a pressure ulcer in her buttock region. To improve quality of life, the patient referral team purchased an air mattress for Leonza from donations made to the general fund.
